Eligibility Information
Individuals must be at least 18 years of age, have a diagnosed intellectual or developmental disability, and an established county case manager who can make a referral for services.
Our Services and Supports
- Individualized service plans that focus on integration, independence, productivity and skill building.
- 24 hour supports provided by highly qualified caregivers certified in First Aid/CPR and OIS to ensure the health and safety of each individual.
- Activities based on the personal preference of each individual.
- Emphasis on community integration through social events, outings as well as employment or alternative programs provided by other community agencies.
- Safe, comfortable, personalized environments tailored to the individual.
- Availability of nursing and behavior consultant services.
